New Patient Registration

At Interlachen Dental Associates, we always welcome new patients.

To save registration time on your first visit, come prepared by completing the Patient Registration and Health History forms. To complete these forms, just print them out, fill in the information, and bring them with you at the time of your appointment.

A copy of our Notice of Privacy Practices are available for you to read and print. At your first appointment you will be asked to sign an acknowledgement that you have received and read the Notice of Privacy Practices.

To print out a complete set of all forms necessary for your first appointment, print New Patient Information forms.
